Your privacy at the clinic

You don't need to describe the problem on the phone, on WhatsApp or on the booking form. The doctor asks the questions he needs in the consulting room, and any examination is done in private.

Erection problems

Difficulty getting or keeping an erection firm enough for sex is common, especially after 40. It can be an early sign of diabetes, high blood pressure or heart disease, so it is worth checking properly rather than treating with tablets bought online. Read about erectile dysfunction treatment.

Early (premature) ejaculation

Ejaculating sooner than you or your partner would like, most of the time, and feeling distressed about it, is the most common sexual complaint in men. Treatment can include simple techniques you practise at home, medicines taken daily or before sex, numbing creams, and counselling, alone or with your partner. Early ejaculation can also go together with erection problems, so the doctor checks for both.

Some men have had early ejaculation since their first sexual experience; for others it starts later. Anxiety about performance, stress, relationship difficulties, erection problems, a thyroid problem and prostate inflammation can all play a part, so when it starts later in life the doctor looks for these first.

Many couples find the stop-start and squeeze techniques useful: stimulation is paused before the point of no return until the urge settles, then started again. Pelvic floor exercises can also help some men gain more control.

Low testosterone (hypogonadism)

Low sex drive, tiredness, low mood, fewer morning erections and loss of muscle can sometimes be due to low testosterone. The diagnosis needs symptoms plus blood tests taken in the morning, usually more than once. Treatment depends on the cause, and men who want children need a different approach, because testosterone from outside can stop sperm production.

  • Low interest in sex, and fewer morning erections
  • Tiredness, low mood or poor concentration
  • Loss of muscle, more belly fat, or loss of body hair
  • Small or soft testes, or swelling of the breasts

Obesity, diabetes, some medicines, pituitary problems and past injury or infection of the testes can all lower testosterone. Testosterone should not be started without tests, and never bought from gyms or online.

Tight foreskin (phimosis)

A foreskin that will not pull back over the head of the penis can cause pain during erections or sex, ballooning while passing urine, and repeated infection or redness under the foreskin. Diabetes can make it worse. Treatment options include a steroid cream, a small operation to widen the foreskin, or circumcision. Read more on our page on phimosis treatment in Gomti Nagar Extension.

Paraphimosis, where a tight foreskin is pulled back and gets stuck behind the head of the penis with swelling, is an emergency.

Curvature of the penis and testicular pain

A new bend, lump or pain in the penis during erection may be Peyronie's disease. Ongoing pain in the testes is covered on the chronic testicular pain page, and a swelling in the scrotum on the hydrocele page. If you are trying for a baby, see male infertility.

Priapism: an erection that will not go down

An erection that lasts more than four hours without sexual excitement, or continues long after sex, is called priapism. It can follow injections for erection problems, some medicines, alcohol or drugs, sickle cell disease, or an injury. The longer it lasts, the higher the risk of lasting damage to the erection tissue. Go to an emergency department straight away; do not wait for the OPD.

If you have had priapism before, the doctor can look for the cause at the clinic and advise how to lower the chance of it happening again.

Looking for an andrologist near Gomti Nagar?

Andrology, the care of men's sexual and reproductive health, is a part of urology. Urologists train in it during their specialist training, so the urologist at Swastik Urology Clinic sees men for erection problems, early ejaculation, low testosterone, fertility, foreskin problems and pain or swelling of the testes. Many men search for an andrologist or a sexologist first. Whoever you see, check their medical qualification and their registration with a medical council. Our article on finding an andrologist in Gomti Nagar explains the difference.

What happens at your visit to the clinic

  1. The doctor asks about the problem, how long it has been there, your health, your medicines and your relationship, in confidence.
  2. He does a short examination, including the genitals, in private.
  3. He may advise blood tests to look for a cause.
  4. He explains the options and starts treatment that is safe with your other medicines.

Tests the doctor may advise

  • Blood sugar and HbA1c, cholesterol
  • Morning testosterone, and other hormone tests if needed
  • Urine test, if there is infection under the foreskin
  • Penile Doppler ultrasound in some men with erection problems or curvature

The doctor will tell you which of these tests you need and where each one can be done.

Treatment options after evaluation

Depending on the cause, options include lifestyle changes, treating diabetes or blood pressure, medicines, counselling, creams, and in some cases a minor operation. The doctor will explain each option and its likely side effects.

When to go to an emergency department

  • An erection that lasts more than 4 hours, with or without sexual desire (priapism). It can damage the penis if not treated quickly.
  • A tight foreskin stuck behind the head of the penis with swelling (paraphimosis)
  • A cracking sound during sex followed by pain, swelling and bruising of the penis
  • Chest pain or breathlessness during sex
  • Sudden severe pain in a testicle

Questions patients ask

Do I have to say what the problem is when I book?
No. Your name, phone number and a preferred day are enough. You can explain the problem to the doctor in private at the visit.
Can I come alone, or with my wife?
Either is fine. Some men prefer to come alone for the first visit; for fertility or relationship concerns, it often helps if your partner comes too.
Are tablets sold online for "sex power" safe?
Many are not. Some contain hidden medicines at unknown doses, which can be dangerous, especially with heart medicines. A doctor can prescribe tested medicines at the right dose after checking they are safe for you.
Is a tight foreskin always treated with circumcision?
No. A tight foreskin in an adult often loosens with a steroid cream used for a few weeks. Circumcision or a smaller operation is considered if the cream does not help, or if there are repeated infections or scarring.
Can early ejaculation be treated?
Yes. Most men improve with techniques practised at home, medicines and, where it helps, counselling. The doctor also checks for erection problems, which often go together with it.
Is testosterone or a supplement from the gym safe?
No. Testosterone and steroids taken without tests and supervision can shrink the testes, stop sperm production and cause other side effects. If you have symptoms of low testosterone, get tested first.
